I can't speak to how it helps with back pain. For me, it was my tail bone that was hurting or uncomfortable. With this pillow, no more discomfort. I tried it on my office chair, but it isn't quite right for me on that - it just didn't sit on the office chair right or something. I haven't tried it in a car, but it seems like it would help there on long trips.
